Reliant at Home provides Home Health, Hospice, Caregivers, and Rehab services to senior adults who need care at home. Care means more to us than just the services we provide. The Bluebird Promise defines our purposeful endeavor to fulfill a higher calling to serve others.

Our Services

Reliant at Home offers a full continuum of home healthcare options to help seniors thrive in the place that matters most to them – their home and community.

Home Health

Home Health includes care provided by a Skilled Nurse (RN/LVN), Skilled Therapist (PT/ST/OT), Medical Social Worker, and Aide. Home Health services are 100% covered by Medicare (no deductible requirements) and some insurances (deductible requirements vary by plan).


Hospice includes care provided by an interdisciplinary team of doctors, nurses, social workers, and counselors that specialize in symptom management, pain control, comfort measures, and end-of-life planning.  Hospice is 100% covered by Medicare.


Caregivers are an extra set of hands to help with personal care, meal preparation, light housekeeping, pet care, and errands.  Customized care plans are paid on a monthly fee schedule and can be customized to fit needs and schedules.  Caregivers services are not covered by Medicare, but some long-term care plans may have provisions to cover or augment costs.


Rehab, also known as Outpatient Rehabilitation Services, includes care provided by a Physical Therapist, Speech Therapist, or Occupational Therapist.  Rehab can be provided at home or in a rehab setting (clinic or gym).  Rehab is covered by Medicare Part B, Medicare Advantage, and Insurance plans and has deductible requirements.
